Provider Plexus takes responsibility for the transition from ED discharge to the visit with our neurology team. We track appointments, procedures, and imaging, help with scheduling, check in with patients, and automatically escalate to our neurology team when needed.

    One secure destination for outpatient neurology referrals from your team, with the referral reason and available discharge information.

  2. Connect & review

    Our team contacts the patient, gathers records, and maps the appointments, procedures, and imaging needed between discharge and neurology follow-up.

  3. Schedule & track

    We help arrange care and track scheduled visits, procedures, and imaging through completion, following up on missed appointments and outstanding steps.

  4. Check in & follow through

    We check in with the patient, automatically escalate to our neurology team when clinically needed, and keep your team informed through the completed handoff.

Before the first referral

Agree on the essentials.

Start with a lightweight workflow. Confirm the operational details together before patients enter the pathway.

Patient fit and timing
Define referral criteria, participating states, available appointments, and cases that need another level of care. No fixed appointment turnaround is promised.
Referral information
Agree on a secure intake channel for the referral and available discharge information. Our team gathers the relevant records needed to review and coordinate care.
The return pathway
Map your existing specialty workflows so we can route patients who need care beyond our services. Agree how findings and referral status return to your team.
Exceptions and escalation
Agree on patient check-ins, criteria for automatic escalation to our neurology team, and contacts for patients who cannot be reached, miss a procedure or appointment, or need more urgent evaluation.
Review and integration
Agree on status updates, completion milestones, and pilot review measures. Assess deeper EHR, API, or record-exchange integration separately based on feasibility and scope.

Outpatient neurology only

This pathway is for patients discharged as appropriate for outpatient follow-up. It does not replace emergency evaluation, inpatient consultation, or the health system’s ongoing neurology care.

We keep planned visits, procedures, and imaging moving toward completion and stay in touch with the patient until the neurology handoff. Clinical concerns automatically escalate to our neurology team when needed.

$39 cash pay per patient visit. No hospital fees during the pilot.
